Can u please add a forms page where u can select what form u want to fill out, and instead of the submissions being in the profile settings it can be in that forms page.

You could already do this yourself. Make multiple forms and then create a new custom page in NamelessMC. You can then just include hyperlinks to those 2 forms so people can choose which one they want to fill in!
