I technically already made this but not using the modals. Reason for this is that the modals are way too limited still and lack a lot of features. You can find the source here https://github.com/supercrafter100/Nameless-Forms-Discord-Bot.

Ive talked about the developer of nameless-link about forms within it but it’s just way too complicated to implement for them
